ESA documentation evaluation — $100 self-pay

The $100 fee is for a clinical evaluation and documentation review. Payment does not guarantee that an ESA letter will be issued.

Ondoc issues ESA documentation only when the treating clinician determines that the applicable clinical and legal requirements are met.

Ondoc clinic policy requires an established clinical relationship and sufficient clinical information before ESA documentation is issued. Additional evaluation, treatment or follow-up may be required, and state law may impose additional requirements.

Ondoc ESA documentation is intended for housing accommodation requests. It does not make an animal a service animal, and it does not grant general public-access or airline rights.

FAQs

Does the $100 fee guarantee an ESA letter?

No. The fee is for a clinical evaluation and documentation review. Documentation is issued only when the treating clinician determines that the applicable clinical and legal requirements are met.

Can I get documentation at my first visit?

Not necessarily. Ondoc clinic policy requires an established clinical relationship and sufficient personal clinical knowledge of you. Additional evaluation, treatment history or follow-up may be required, and state law may impose further requirements.

Does it work for flights or public places?

No. It is for housing accommodation requests only. See the ESA documentation policy for details.

How long is documentation good for?

Ondoc clinic policy is that documentation is never renewed automatically — clinical reassessment is required before updated or renewal documentation is issued. State law may impose its own reassessment requirements, and housing providers may ask for current documentation.

What about a cat, dog or another animal?

The clinician evaluates a disability-related need for the specific animal you describe. Each animal requires its own clinical determination; one determination is not assumed to cover additional animals.

How ESA documentation evaluations work
