ESA documentation policy

Ondoc supports patients who need appropriate housing accommodations. What you book is an ESA documentation evaluation at $100 self-pay, or commercial insurance where applicable. You are not purchasing a letter, and payment does not guarantee that documentation will be issued.

What an ESA documentation evaluation is

Ondoc clinic policy

You are booking a clinical evaluation with a licensed Ondoc clinician, priced at $100 self-pay. Commercial insurance may be used where applicable and subject to verification.

You are not purchasing a letter. The clinician decides, independently, whether ESA documentation is clinically and legally supportable. If it is not, no documentation is issued.

Established clinical relationship

Ondoc clinic policy

Ondoc does not operate as an administrative, one-visit ESA letter service. ESA documentation may be issued only within an established clinical relationship, where the clinician has sufficient personal clinical knowledge of the patient to support the determination.

A one-time administrative request, by itself, is not sufficient. Additional evaluation, treatment history, follow-up or longitudinal care may be required before documentation can be issued. We do not promise that documentation can be issued at a first visit.

Qualifying disability and functional impact

Applicable legal framework

Housing accommodation frameworks require that the person have a disability and a disability-related need for the assistance animal.

Before issuing documentation the clinician must determine that the clinical record supports a qualifying disability, meaningful functional limitation, and a disability-related need for the animal. A questionnaire score never qualifies a patient automatically, and no diagnosis is made merely to produce documentation.

Disability-related therapeutic benefit

Ondoc clinic policy

The clinician must independently determine that the animal provides a disability-related therapeutic benefit or assistance relevant to your functioning, and that determination must be supportable in the medical record.

There is no automatic rule. General stress, convenience, companionship preference, or a wish to avoid ordinary pet fees does not by itself qualify anyone.

Scope: housing accommodation requests only

Ondoc clinic policy

Ondoc's ESA workflow is intended for housing accommodation requests.

ESA documentation does not make an animal a service animal, does not grant general ADA public-access rights, does not guarantee access to restaurants, stores or workplaces, and does not grant airline service-animal status. It also does not guarantee that a housing provider will grant the accommodation.

Provider qualifications and independent judgment

Ondoc clinic policy

Before issuing ESA documentation the clinician must affirm that they are appropriately licensed for the encounter and patient jurisdiction, that issuing the documentation is within their professional scope, that applicable state-specific requirements were considered, that a sufficient clinical relationship exists, and that they personally support the determination.

No administrative staff member and no automated workflow can override or substitute for the clinician.

State requirements

Applicable legal framework

State requirements for assistance-animal documentation differ, and may address the provider-patient relationship, licensure and jurisdiction, telehealth prerequisites, required notices and reassessment.

Ondoc does not publish a state-by-state legal matrix and does not claim compliance with every state's requirements on the basis of this page. The treating clinician reviews and confirms the applicable state requirements for your jurisdiction before any documentation is issued.

Reassessment and renewal

Ondoc clinic policy

ESA documentation is not renewed automatically. Ondoc requires clinical reassessment before issuing updated or renewal documentation, and a renewal requires a new clinician determination — a prior clinician's attestation is never copied forward.

Applicable state law may require a particular reassessment interval or additional renewal requirements. An existing housing accommodation does not automatically become legally invalid on an anniversary date; a housing provider may nonetheless ask for current documentation.

Requests Ondoc will not complete

Ondoc clinic policy

Ondoc clinicians will not knowingly issue ESA documentation where the request is for a non-housing purpose under this workflow; adequate clinical support is absent; sufficient clinician-patient relationship or personal knowledge is absent; the request is solely administrative or convenience-driven; the request is solely intended to avoid ordinary pet costs without a qualifying disability-related need; the documentation would misrepresent the animal as a service animal; the representation would exceed the provider's scope; applicable state requirements have not been satisfied; or the clinician cannot independently support the determination.

In those situations the clinician can still continue treatment or arrange a referral as clinically appropriate.

Privacy and third-party verification

Ondoc clinic policy

Ondoc has no automated landlord, property-manager or screening-company verification endpoint, and no clinical information is disclosed automatically to anyone who contacts us.

Verification requests are handled by an approved staff or provider workflow, require appropriate patient authorization or another documented lawful basis, and are limited to the minimum information authorized. We do not routinely disclose diagnosis, severity, records, therapy notes or medication history, and a third party's own form does not expand what we will disclose.

What you confirm before booking

I understand that the $100 fee is for an ESA clinical evaluation and does not guarantee that an ESA letter will be issued. I understand that additional clinical care or follow-up may be required before documentation can be provided.
