ESA documentation for housing — how Ondoc handles it
Housing accommodation frameworks let a person with a disability request a reasonable accommodation, including for an assistance animal. Supporting documentation comes from a treating clinician who can speak to a qualifying disability and a disability-related need for the animal.
ESA documentation evaluation — $100 self-pay
The $100 fee is for a clinical evaluation and documentation review. Payment does not guarantee that an ESA letter will be issued.
Ondoc issues ESA documentation only when the treating clinician determines that the applicable clinical and legal requirements are met.
Ondoc clinic policy requires an established clinical relationship and sufficient clinical information before ESA documentation is issued. Additional evaluation, treatment or follow-up may be required, and state law may impose additional requirements.
Ondoc ESA documentation is intended for housing accommodation requests. It does not make an animal a service animal, and it does not grant general public-access or airline rights.
What the evaluation covers
- $100 self-pay for the clinical evaluation, or commercial insurance where applicable
- Ondoc clinic policy: documentation requires an established clinical relationship and sufficient clinical information — a one-time administrative request is not enough
- The clinician must independently determine a qualifying disability, meaningful functional limitation and a disability-related need before any documentation is issued
- Ondoc cannot guarantee that a housing provider will grant the accommodation
