ESA verification requests — how Ondoc responds
If a landlord, property manager, housing provider or screening company contacts Ondoc about a patient's assistance animal, no clinical information is released automatically. Every request goes through an approved staff or provider workflow.
ESA documentation evaluation — $100 self-pay
The $100 fee is for a clinical evaluation and documentation review. Payment does not guarantee that an ESA letter will be issued.
Ondoc issues ESA documentation only when the treating clinician determines that the applicable clinical and legal requirements are met.
Ondoc clinic policy requires an established clinical relationship and sufficient clinical information before ESA documentation is issued. Additional evaluation, treatment or follow-up may be required, and state law may impose additional requirements.
Ondoc ESA documentation is intended for housing accommodation requests. It does not make an animal a service animal, and it does not grant general public-access or airline rights.
What the evaluation covers
- There is no automated landlord or third-party verification endpoint at Ondoc
- Appropriate patient authorization or another documented lawful basis is required before any disclosure
- Only the minimum authorized information is disclosed — never diagnosis, severity, records, therapy notes or medication history
- A third party's own form does not expand Ondoc's approved disclosure scope
Frequently asked
Will Ondoc fill out a screening company's questionnaire?
Only within our approved disclosure scope and with appropriate patient authorization. We do not answer requests for clinical detail beyond what is authorized and appropriate, regardless of what the form asks for.
Can Ondoc confirm my documentation to my landlord?
A verification response can be arranged through our staff workflow with your authorization, limited to the minimum authorized information.
