An IBS management visit confirms the diagnosis (Rome IV criteria), classifies the subtype (IBS-D, IBS-C, IBS-M), and rules out mimics such as celiac disease, inflammatory bowel disease, microscopic colitis, and bile-acid diarrhea. Foundational management addresses diet (often a structured low-FODMAP trial), fiber, hydration, sleep, and stress, with targeted medications based on subtype: antispasmodics, peppermint oil, eluxadoline or rifaximin for IBS-D; linaclotide, plecanatide, or lubiprostone for IBS-C. Gut-directed cognitive behavioral therapy and hypnotherapy are evidence-based adjuncts.
