Metformin
The standard first-line oral medication for type 2 diabetes.
Metformin lowers blood sugar primarily by reducing how much glucose the liver produces. It's weight-neutral or mildly weight-lowering, very inexpensive, and the only oral diabetes drug with strong long-term safety data. The main side effect is GI upset, which usually settles with slow dose escalation and taking it with food.
