Bladder pain syndrome/interstitial cystitis (BPS/IC) is a chronic condition marked by pelvic pain related to bladder filling, urinary urgency, and frequency, lasting more than six weeks without infection or other identifiable cause. The underlying mechanism is incompletely understood and likely involves a defective urothelial barrier, neuroinflammation, and pelvic floor dysfunction. Management is stepwise: dietary trigger avoidance, stress management, pelvic floor physical therapy, oral medications such as amitriptyline or pentosan polysulfate, bladder instillations, and — in refractory cases — neuromodulation or specialist procedures.
