Will the antibiotic be covered under Empire Plan?
First-line UTI antibiotics (nitrofurantoin, TMP-SMX, fosfomycin) are on virtually every Empire Plan formulary — typically Tier 1 generics.

Self-pay is available for the Ondoc visit, and you choose whether to continue before any payment.
