Bring your school or league form and the clinician works from it directly. If your form requires additional measured information that has to be obtained in person, the clinician tells you exactly what is needed and how to get it, and may ask for an in-person evaluation before signing — signing stays a clinical judgment bound by your specific form. Ondoc is a virtual practice and does not operate a local clinic.

About School / sports physical
A pre-participation sports physical screens for conditions that increase the risk of injury or sudden death during athletic activity. It reviews family history of premature cardiac death or inherited cardiac disease, prior injuries, concussion history, asthma, allergies, and menstrual or eating concerns. Screening attention centers on the cardiovascular and musculoskeletal systems, with a brief neurologic and vision review. Most clearances are unrestricted; selected findings warrant further testing — such as an EKG for cardiac red flags — or modified clearance, and are documented on the state-required form. This evaluation is performed by telehealth, and the clinician completes the form when its requirements can be satisfied remotely; when a form calls for findings that cannot reasonably be obtained remotely, additional measured information or an in-person evaluation may be required first.
