A menopause consultation assesses vasomotor symptoms, sleep, mood, sexual health, bone and cardiovascular risk, and personal and family history that would influence hormone therapy. Systemic estrogen — with progesterone in patients with a uterus — is the most effective treatment for hot flashes and night sweats in appropriate candidates within 10 years of menopause and under age 60. Non-hormonal options include SSRIs/SNRIs, gabapentin, oxybutynin, and the neurokinin-3 antagonist fezolinetant. Low-dose vaginal estrogen treats genitourinary syndrome locally with minimal systemic absorption.
