The list of medications your insurance plan covers, grouped by cost tier.

Each insurance plan has its own formulary that sorts drugs into tiers — generics on the cheapest tier, brand-names higher, and specialty drugs highest. Drugs not on the formulary either aren't covered or require prior authorization. Checking the formulary first can save weeks of back-and-forth.
