EHR (Electronic Health Record)
Also called: EMR, electronic medical record
Your digital medical chart maintained by a clinic or hospital system.
An EHR stores your visits, labs, imaging, medications, and provider notes. When you switch providers, sending records or using an interoperability network (like Carequality or TEFCA) lets a new clinician see your history without starting from scratch.
