Prior authorization

UHC typically requires PA for lama/laba combinations prescribed for copd. Ondoc's clinician completes the PA form and submits it directly to UHC — you don't fill out any paperwork.

UHC requires PA for GLP-1s, controlled stimulants, and specialty drugs. Our clinician submits the PA to OptumRx.

California state mandate

California follows standard commercial-formulary rules for copd. No state-specific mandate expands coverage beyond UHC's national formulary at this time. Federal ACA preventive-services rules still apply where relevant.
