The preventive/wellness portion of an annual check-up is often covered at $0 by BCBS FEP when the clinician participates in your plan. Any lab work is billed to your BCBS FEP lab benefit.

Yes. The clinician orders labs at Quest or LabCorp and imaging at national facilities so your plan's lab/imaging benefit applies.
