BCBS typically requires PA for lama/laba combinations prescribed for copd. Ondoc's clinician completes the PA form and submits it directly to BCBS — you don't fill out any paperwork.

Most BCBS plans require PA for GLP-1s, brand stimulants, and specialty drugs. We submit PAs to the correct regional Blues processor.

Texas state mandate

Texas follows standard commercial-formulary rules for copd. No state-specific mandate expands coverage beyond BCBS's national formulary at this time. Federal ACA preventive-services rules still apply where relevant.
