Rescue inhalers are Tier 1. Combination ICS/LABA inhalers often require step therapy; biologics (Dupixent, Xolair) require PA.
BCBS formularies vary by state plan (Anthem, Highmark, Florida Blue, Empire, BCBS TX, and others). We check your specific plan's formulary before approving the refill.
BCBS typically requires PA for ics, laba, biologics prescribed for asthma. Ondoc's clinician completes the PA form and submits it directly to BCBS — you don't fill out any paperwork.
Most BCBS plans require PA for GLP-1s, brand stimulants, and specialty drugs. We submit PAs to the correct regional Blues processor.
California follows standard commercial-formulary rules for asthma. No state-specific mandate expands coverage beyond BCBS's national formulary at this time. Federal ACA preventive-services rules still apply where relevant.
